PRIVACY: We collect information from you when you schedule a free consultation, when you book a service package, or fill out a form. You may be asked for you name, e-mail address, phone number or credit card information. When completing a "client intake" or "consultation" form, you may be asked for personal information including, but not limited to: your age, gender, name of your healthcare provider, a brief medical history. This information is collected with the purpose of providing informed and adequate care. This information is disclosed confidentially with Onyx Doula, and is shared with your doula, your medical staff and a back-up doula where applicable. Your information, whether public or private, will not be sold, exchanged, transferred, or given to any other company for any reason whatsoever, without your consent, other than for the expressed purpose of delivering the purchased product or service requested.
Note: The scheduling of a free consultation through this website does not guarantee an in-person meeting with a doula. A free consultation may be done via text, voice or video call and may be cancelled at anytime by both Onyx Doula, or the scheduling party. Also note there is one free consultation provided per service purchase. In addition, any and all prices listed on this website for any products and or services, are subject to change at any time without notice.
